To obtain a Rare Books/Special Collections/University Archives user card, please:
  1. Complete the following form, THEN
  2. Go to the Rare Books/Special Collections desk to activate your card.

You must present photo ID including your current mailing address. Your ID may be photocopied by staff for security reasons.

This card allows use of materials in the Rare Books & Special Collections / University Archives Reading Room ONLY.

By applying for this card you are agreeing to use Library materials in the Rare Books & Special Collections / University Archives Reading Room and to return them by the due date/time. The information on this application form is collected under authority of the University Act RSBC 1996, c.468.s27(a) and is required to ensure the security of the UBC Rare Books & Special Collections or University Archives collections. It will be used only to record what materials you have signed out, and, if necessary, will be used to identify users in the event of loss, damage or theft of library materials. The information on this form will be transferred to a secure electronic Library database.
